Name Change Requests

If your name has changed due to marital status you may write us and include a copy of your Driver’s License (with your current name on it) or a copy of your Marriage License, or you can come into our office and we will make a copy of your Driver’s License (with current name), or Marriage License.

Adding a spouse to your account can be accomplished by writing a letter to the customer service department including your spouse's name and social security and drivers license numbers.

In order to remove a spouse's name from an account, a letter including the signature of both parties is required. In the case of a deceased party a copy of the death certificate, including a letter from the surviving party, is required.

Please include this completed form with any correspondence.
